What is Subcision?

What does Subcision entail?

How can an individual know if Subcision is needed?

A person who has multiple boxcars (rounded craters in the skin with sharp vertical edges) or rolling scars (pits in the skin with sloped sides) that do not disappear upon skin stretching or get smoothened upon smiling require subcision. It is also often carried out in case of very little tethering to acne scars. 

What to expect during the treatment?

Before treatment

During procedure

The dermatologist inserts a tri-beveled needle (18 or 20 number gauge) or a Nokor needle (with a small sharp blade at the tip of a short fine needle) into the skin dermis horizontally (at an acute angle) just below the scar. It is gently moved parallel to the superficial skin just underneath the scar in a back and forth, fan-like motion to take out the complete scar which is connected to its underlying skin tissues. The needle is again moved in the same way after rotating it by 90 degrees. This is called fanning subcision. Lastly, the needle is taken out and circumferentially squeezed back around the point of exit of needle insertion so that a large hematoma is not formed because of bleeding.  

In the case of small areas which require subcision, Cannula is used that is inserted through a tiny hole made by a needle. A scar can be taken out either by carrying subcision in one direction or by creating a crosshatch.

It is an outpatient procedure. Best results are visible in 1 to 3 months as the skin continues to remodel itself. The person generally requires multiple sessions with a spacing of at least 4 weeks. The number of repetitions needed depends upon the shape and size of the scars and the surgical instrument used.

Post-treatment

Immediately after the treatment, the wounded site after needle insertion is healed by applying ice or manual pressure for several minutes. This way the inflammation and bleeding can subside. A person may apply makeup to camouflage the bruised area.
